CareOne Medical Group’s Developmental Disability Program is known as CareOne Group. CareOne Group is an approved provider for DD Medicaid Waiver services for the State of Nebraska.

Supported Family Services

Client support in the family home
Client driven services
Teaching tasks designed to build independent living skills
Staff assist with activities of daily living, health maintenance, social and leisure skills and supervision

Shared Living Services

24-hour client support in another family’s home
Family members of each home go through extensive background checks
Family members are trained and monitored by CareOne staff
Clients matched to families based on individual need
Client driven services
Provider teaches skills for independent living and helps you with activities of daily living, health maintenance, social and leisure skills and supervision

Independent Living Services

Intended for clients that are living independently in the community
Hourly services provided in your own home or apartment
Skill training based on client need and desires
Habilitative community inclusion included

Respite Services

Short term hourly services when you need a break
Temporary relief for the daily caregiver
Staff provide assistance with activities of daily living and supervision
Limited to 240 hours per year for DDAD Waiver and 360 hours per year for CDD Waiver
Maximum of 8 hours per day limit

Supported Employment Services

Pre-Vocational Services: Assistance with job preparation, job seeking and all pre-employment services
Job Coaching: One-on-one assistance and help learning the job
Follow-Along: Services to assist the client with re-training and long-term support
Partnering with Nebraska Vocational Rehabilitation to provide employment services beyond the DD population.
